The Absorptive Surface Area of Small Intestine in Digestive Disease in Korean


Abstract
The diagnosis of primary malabsorption syndrome as tropical sprue or gluten-induced enteropathy is usually based on the biopsy demonstration of a damaged small intestinal mucosa.
However, several investigators have observed a mucosal lesion in digestive diseases, yet there have been few reports of other conditions with absence of villi and small intestinal mucosal damage that are morphologically indistinguishable from the sprue lesion but apparently etiologically distinct.
The present study was undertaken in an attempt to establish whether or not jejunal mucosal specimens from the patients with digestive diseases were indeed damaged in nature, as well as to investigate the histochemical response to the damaged jejunal mucosa.
The results obtained are as follows:
1) In normal Korean subjects, the average dimensions, measured within a field0OX10) were: 443P in villous height, 136p in villous width, 122p in mucosal thickness, 125p in crypt depth and 24,u in height of columnar epithelial cell. Besides, the number of villi in a visual field were 8 and there was a mild lymphocytic infiltration in columnar epithelial cells in 43% of the normal subjects.
2) With 207 cases of digestive diseases in Korean, we observed mucosal lesion to a considerable extent in. the majority (95%) and the so-called subtotal atrophy of the mucosa in 11.6%, unmeasured cases included, which is supposed to be identical) with those findings in sprue.
3) In disease group, the over-all averege dimensions were: 263p (59l against 100% in control) in villous height, 1.63,u (112%) in villous width, 204p (122%) in mucosal thickness, 124P (991) in crypt depth, 2e,u (1160) in height of columnar epitbelial cell and the number of villi were 6 in each field. In addition to the above findings, we also observed edema (78%), extrayasatedi red, cells(14%) in the mucosa, cell infiltration into columnar epithelium(98%) and abnormality of nucleus and disappearance of basement membrane of columnar epithelial cells.
4) Alkaline phosphatase¢¥activity showed a tendency which runs parallel with the severity of mucosal damage.
